INSURANCE INFORMATION

At the time of your appointment, we will provide you with (1) a physician's referral form including diagnosis obtained from your referring physician and (2) a billing statement from our practice with appropriate diagnosis, treatment and billing codes. Submit these materials to your insurance carrier. We cannot be certain if your particular plan provides coverage and reimbursement for medical nutrition therapy counseling.  Much of it depends on your diagnosis.  We are not contracted with insurance companies yet our patients find it worthwhile to apply for reimbursement.

Medicare places severe restrictions on our counseling protocol and would prevent us from practicing in a manner that would provide optimal outcomes.  We have therefore chosen to "opt out" and are not Medicare providers.
